ADOBE VETERINARY CENTER’S EQUINE WELLNESS PROGRAMS

Open enrollment for 2024 is from january 1 to january 31!

Maximizing your horse’s health and your budget

Our Equine Wellness Programs are a way for you to prepay for a year’s worth of preventative care at a significant cost savings.

Preventive medicine is an important aspect to your horse’s health. With our wellness programs, your horse’s basic healthcare will be covered. An important aspect of our program is that your horse will be examined by one of our veterinarians twice a year. This allows our veterinarians to get to know your horse while he or she is healthy. These wellness exams help us to diagnose early problems or trends before they become big problems. This is also an opportunity to discuss preventive medicine topics such as parasite control, nutrition, hoof care, dental care, weight control and sand prevention.
Our equine program offers 4 different plans to choose from, our Basic Plan, our Basic Plan PLUS Dental, our Premium Plan, and our Premium Plan PLUS Dental, each offering a different level of preventative care designed to meet your specific needs. Program Price is per horse and represents a minimum of 15% cost savings.

Our Wellness Plans

Our Basic Wellness Plan Includes

includes:

• Two comprehensive physical exams with a full report card of the findings and recommendations for your horse’s health – Spring and Fall
• Spring and Fall vaccines which typically include annual West Nile Virus, Eastern, Western and Venezuelan Encephalitis, Tetanus, Rabies and biannual Influenza and Rhinovirus. Not all horses require the same vaccines so the veterinarian will tailor the vaccines to your individual horse.
• A parasite program that includes up to 2 fecal parasite exams and a deworming product if needed for your horse.

Our Basic Wellness Plan PLUS Dental

includes:

• All of the components of the Basic Plan plus a routine dental float and the associated required sedation. Extractions or extensive corrections are not included.

Our Premium Wellness Plan

includes all the benefits of the Basic Plan plus:

• A Complete Blood Count and Comprehensive Blood Chemistry Panel with electrolytes to establish your horse’s baseline values and identify any early problems.
• Baseline digital radiographs (4 total) of your horse’s front feet. These help to identify signs of imbalance or laminitis. These images will be available to us as a reference point if your horse ever has signs of laminitis or other foot problems. We can email copies of these images to you and your farrier if you would like.
• A Coggin’s Blood Test for Equine Infectious Anemia.

Premium Wellness Plan PLUS Dental

• All of the components of the Premium Wellness plan plus a routine dental float and the associated required sedation. Extractions or extensive corrections are not included.

Equine Wellness Program FAQs

ADOBE VETERINARY CENTER’S EQUINE WELLNESS PROGRAM
No. Our Wellness Plans are not insurance plans, and no deductibles are involved. The Wellness Plans are meant to ensure that your pets receive the annual preventive care they need, which allows them to live a long and happy life.
If you don’t use all the services offered in the Wellness Plans, they will expire when the Plan expires. There are no substitutions or refunds given on unused services. It is up to you to bring your pet in for their services before the Plan ends. Any services and products not included in the wellness plans will be an added cost to the owner.
Our Wellness Plans cover preventive care for your horse. If your horse becomes sick; injured; or requires any service, product, or medication not covered under the Wellness Plan, those extra services and products will be an additional cost to you, the owner.

You may cancel your Wellness Plans before the end of its term. We will give a refund minus the full retail value of the services that have already been given.

Wellness Plans are not transferable to another horse. If you horse is sold before fulfillment of the wellness program, the remaining services of the program may be transferred with the horse and be available for use by the new owner if they are located within the Adobe Veterinary Center practice area or if the new owner chooses to haul the horse in for these visits. No refunds or substitutions will be made for unused opportunities.
In the event your horse should pass away before fulfillment of the wellness program, we will alter your account on a pro-rated basis. We will calculate the full retail value of the services covered in the Plan that you have used, minus the payments you’ve already made and adjust the costs accordingly. Depending on how many services you have used and how many payments you have made, we may owe you money or you may owe us money.

We are only allowing full payment at time of enrollment, from January 1, 2024 – January 31, 2024. Payments can be made with a credit or debit card and checks. Care Credit cannot be used for Wellness Plans.
If for any reason your card is declined on the day your payment is scheduled, someone from our Team will notify you within 48 hours, and a $25 fee will be charged in addition to the amount of your monthly payment. We will continue to attempt to process payments on that same card if we don’t hear from you. If a second payment is missed, and we cannot reach anyone to update the payment method, the Plan will be terminated. You, the pet owner, are responsible for paying the remaining balance on your Plan or the full retail value of the services utilized in the Plan, minus the payments you’ve already made – whichever ends up being less. If the remaining balance of your Wellness Plan is not paid within 30 days of the Plan’s date of termination, the balance due will be submitted to our collections department.
Yes, Wellness Plans make great gifts. The recipient of the Wellness Plan must be in agreement, and he or she is required to agree to the Terms and Conditions of our Wellness Plans.

Please read and initial all lines and sign the bottom of this sheet.

Horses must be at least one year of age to be enrolled in the program.
Package fee must be paid in full.
In order to provide cost savings to our owners, any outstanding account balances must be paid in full before enrollment in the program.
An overdraft and reprocessing fee of $35 will be charged for any rejected charges.
Care Credit is not accepted for this program.
The renewal date is between January 1st and January 31st annually.
Barn calls fees are NOT included in the price of the program and must be paid for at the appointment. You are welcome to haul your horses to the clinic to avoid barn call fees.
Any additional services provided at the time of the wellness visit will be charged for and are payable on the date of the visit. This includes upgrades to full exams for horses that present with issues at their wellness exam requiring additional time.
We strongly recommend that owners be present for all appointments. For the provision of services, written or verbal authorization is required by the owner if they cannot be present when services are rendered.
Vaccines given for the first time require boosters in 3-4 weeks. Boosters for initial vaccination series are not included in the program and are charged separately. Initial boosters will require a separate visit.
The program covers the first two fecal parasite exams and the first deworming product. Additional fecal parasite exams or dewormers are not covered by the plan. Due to the fact that we tailor parasite control programs differently for each horse, not all horses will need two fecal parasite exams and a deworming product. Habronemiasis treatment does not qualify for this deworming benefit.
Owners are responsible for contacting the office to schedule appointments.
If you do not utilize all the services included in your plan, there is no refund or substitution made on your unused opportunities.

No refunds or substitutions will be made for unused opportunities.
In the event your horse should pass away before fulfillment of the wellness program, we will alter your account on a pro-rated basis.
If an owner’s account does not remain at a zero balance, any paid program monies may be utilized to bring the account current and the horse will be dropped from the program.
The blood work and radiographs included in the Premium Plan must all be done on the same day when the horse is healthy.
